INQUIRY LABORATORY RESULTS
Please supply the following data:
1. Lab number
2. Name of patient
3. Date of request
4. Laboratory test asked to perform
5. Name of Inquirer
REQUEST LABORATORY TEST
1. Specimens should be properly collected & prepared in their appropriate containers, which must be clearly labelled and identified.
2. The request form must accompany each specimen. It should be properly completed and signed by medical officer or authorized person.
3. Please ensure that there is no leakage from the specimen containers. For safety reason, leaking specimen or soiled request form will not be processed.
REQUEST FORM
All request forms should have the following information properly filled in :
1 Hospital number
2 Hong Kong ID number or Passport number
3 Patient's name, sex and age
4 Clinical history or findings
5 Doctor's name
6 Doctor's signature (if required)
7 Nurse's signature (if in Hospital wards)
8 Requesting ward/unit and report location
9 Type of specimen
10 Test requested
11 Degree of Urgency
Attached the LABEL bearing the above information in the assigned position. If writing is necessary, please write legibly. Different laboratory forms are used for different tests :

SPECIMEN CONTAINERS | |||
1. | PLAIN BLOOD TUBES: 10 ml (13 X 100 mm) PVC tubes - RED caps with gel (Greiner). Mixed gently after collection. | ||
2. | HEPARINIZED BLOOD TUBES: 5 ml (18 x 60 mm) PVC tubes - ORANGE caps with beads (Sarstedt). Mixed gently after collection. | ||
3. | EDTA BLOOD TUBES: 5 ml (18 x 60 mm) PVC tubes with RED caps (Sarstedt). Mixed gently after collection. | ||
4. | CITRATED BLOOD TUBES : Exact 2 ml blood in PVC tubes with LIGHT BLUE caps(Greiner). Mixed gently after collection. | ||
5. | URINE CONTAINER: 70 ml sterile PVC bottles with YELLOW caps |

6. | STOOL CONTAINER: 70 ml sterile PVC bottles with RED caps | ||
7. | STERILE CONTAINERS FOR MICROBIOLOGY INVESTIGATION: please refer to Microbiology section. | ||
REJECTION OF SPECIMENS | |||
Under following conditions, the requisition of laboratory tests will be rejected: | |||
a) | Tests not available | ||
b) | Concerning the specimens: | ||
i) | Not labeled or improperly labeled | ||
ii) | Outdated | ||
iii) | Specimen container contaminated with blood or body fluid | ||
iv) | Improperly collected | ||
c) | Concerning the request form | ||
i) | Inadequate patient data | ||
ii) | No indication of tests requested | ||
iii) | No indication of type or nature of specimen | ||
iv) | Discrepancy in labeling between the form and specimen | ||
HIGH-RISK SPECIMENS | |||
Specimens from certain patients are highly hazardous to staff. Such patients are: | |||
a) | Known to be positive for HIV antibody or HBsAg | ||
b) | Diagnosed viral hepatitis | ||
Specimens should be placed in a sealed leak-proof plastic bag with BIOLOGICAL HAZARD label. | |||
SENDING & COLLECTION OF LABORATORY REPORT FORMS | |||
Send forms to the reception desk of the laboratory. Do not dirty or damage the lab forms. PLEASE SIGN BEFORE COLLECT ANY REPORT FORMS FROM THE LABORATORY. All reports will be directed to the requesting doctor without any delay. Report forms will also be sent through Fax if requested during requisition. Copies of laboratory reports are obtainable from the hospital record room at minimal charge. Please contact our staff in General office. Requisition of copies/ duplications of laboratory report will usually not be entertained by laboratory unless sound reason is given. According to the Patient's Right Ordinance, messenger, Amah, Nurse and other hospital staff should handle patient reports with care & all results should be treated as confidential information. |
