微生物
一般資料
1)一般微生物化驗樣本和化驗表單,請依指示送到化驗室。如有疑問，請向化驗室工作人員查詢
2)每個樣本應妥善收集，放置在適當的容器中, 並加蓋緊
3)所有被視為具有潛在傳染性的樣本必須小心處理，以避免污染樣本容器外。所有樣本容器必需蓋緊，否則樣本將被拒絕接收並退回所發送到部門
4)每個樣本必須附有適當的化驗表單,所有提供資料必須正確填寫於表單上
5)請發送所有收集樣本往化驗室
6)化驗表單
a) 小便常規，妊娠試驗，大便常規，大便潛血及糞便對輪狀病毒請使用淺黃色的化驗表單
b) 所有培養細菌，塗片檢查和病毒抗原檢測用深黃色的化驗表單 .化驗表單上請填寫病人正服用的抗生素藥名稱,以方便試驗.不用填寫抗菌劑，如抗真菌，抗病毒等藥物的名稱
c) 體液常規，腦脊液常規和腦脊液化驗請使用綠色化驗表單
d) 病毒研究，病毒培養需發送（衛生署）病毒科作調查，請附上額外的粉紅色申請表單（DH1293A）主診醫生亦請正確簽署和填寫病人臨床資料和過往病歷紀錄
